HOW TO SUBMIT A NEW APPLICATION:
- Submit application online by clicking the "Submit by Email" button.

If your email provider does not support this option, you can send it as an attachment to linda.gosdeck@midwesthorsefair.com.
1. Save the Application to your computer.
2. Fill in the Application.
3. Save the Application once it has been filled in.
4. Email to linda.gosdeck@midwesthorsefair.com. - You may also fax or mail it to the address located at the bottom of the application.
Please note that if you do not receive a confirmation email, then your application was not received. If you have any questions please call 920.623.5515. Please make sure that you read and understand all of the information before submitting an application.

PARTICIPATING EXHIBITORS:
- Please send back your signed Exhibitor Booth Contract by February 1, 2013. This form can be mailed or faxed to 920.623.5454.
- A Certificate of Liability Insurance is also due in our office by February 1, 2013. More details can be found in the Rules & Regulations book.
